Lead Gen Forms are templates that can be attached to a single image, carousel image, video, message, document, or conversation ad to collect information from the member if they click on the call-to-action button. The form is automatically filled with the member’s contact and profile information.

Form name: Cannot be left blank. Use up to 256 characters to name your Lead Gen Form. The form name is only displayed in Campaign Manager.

Language: The form languagemust match the language you've chosen for your campaign.

Offer headline: Cannot be blank. Use up to 60 characters.

Offer details: Use up to 70 characters to avoid truncation (160 max characters).

Privacy policy URL: Cannot be blank and must start with http:// or https://. Use this field for a webpage that describes your company's privacy policy and what you will do with the information a member chooses to submit to you. Learn more about the Lead Gen Form Privacy Policy.

Privacy policy text (optional): Use up to 2,000 characters to explain how you’ll use the member’s data and link to your company's privacy policy.

Profile information: Select up to 12 profile demographic question fields. First name, Last name, and Email address are automatically selected, but you can unselect them if required.

Custom questions (optional): Add up to three optional Custom questions. Each field has a maximum of 100 characters.
- Custom questions count toward the 12-question maximum for your form.
- Custom questions are required fields a member needs to answer to submit the form.
- Fields are supported by most marketing automation/CRM partners.
- Multiple choice questions allow members to choose from a list of answers you define.
- Single line text questions allow members to enter a free-text answer.

Custom checkboxes (optional): Choose up to five optional Custom checkboxes to get members’ specific consent for additional ways you intend to use their information.
- Custom checkboxes don’t count toward the 12-question maximum for your form.
- The Checkbox message has a maximum of 500 characters.
- You can make Custom checkboxes required or optional fields for the member to complete.

Message: Cannot be blank. Use up to 300 characters to write a thank you message to members who submit a response to the Lead Gen Form.

Landing page URL: Cannot be blank and must start with http:// or https://. Use this field to provide a landing page to members who submit a response to the Lead Gen Form. If you linked to a whitepaper or other virtual asset, you’ll want to include an ungated link to it so members can access it. LinkedIn does not automatically send any assets to members.

Thank you message call-to-action: Choose a call-to-action from the dropdown menu. The call-to-action selected will link to the landing page URL.
